OnTrackNY teams support the well-being of teens and young adults across New York state who are impacted by unexpected changes in thinking and perceptions. Each specialized team offers a comprehensive set of support services, also known as Coordinated Specialty Care (CSC), that aims to empower OnTrackNY participants to make meaning of their experiences and pursue their goals for school, work, and relationships.  Research has shown that this type of early intervention, considered to be the gold standard model of care for early psychosis, can make a dramatic difference in young people’s lives.

We place participants at the center of everything we do and focus on helping them pursue their life goals. Our services are offered regardless of insurance status or ability to pay.

The OnTrackNY spirit

Everything we do is guided by 4 values.

Recovery-oriented

We view recovery as a process of change, supporting participants as they strive to improve their health, live a self-directed life, and reach their full potential.

Equitable & inclusive

We value each other’s differences as strengths and aim to ensure all participants and families have what they need to thrive wherever they live, work, learn, or play.

Collaborative

We partner with participants & their family members to navigate and make decisions in all aspects of the treatment journey.

Person-centered

We place participants at the center of everything we do, prioritizing their unique goals, needs, and preferences.

We’re [serving] young people who’ve been diagnosed fairly recently because research suggests that the sooner someone is treated, the better their prospects are for recovery.

- Lisa Dixon, MD, MPH, Director at Center for Practice Innovations (CPI) and OnTrackNY
