OnTrackNY teams support teens and young adults in New York state who are between 16-30 years old and have been recently impacted by unexpected changes in thinking and perceptions. Each specialized team offers Coordinated Specialty Care (CSC), a comprehensive set of support services that are grounded in research and guided by the goals, needs, and preferences of each individual participant. Participants can choose to engage in the elements of our services that best align with their aspirations for school, work, and relationships.

OnTrackNY CSC services are offered regardless of insurance status or ability to pay and include:

Individual therapy

Participants are offered cognitive behavioral-based therapy with the Primary Clinician to help them understand their experiences and develop strategies to maintain wellness.

I have a whole team behind me who wants to help me go to school, help me go to work, help me get out of bed in the morning when I can't. Today, I have a lot more hope that I can live a full life. Even if days are hard, my future is in my control now -- before it didn't seem that way.
