OnTrackNY supports teens and young adults across New York state, ages 16-30, experiencing unexpected changes in thinking or perceptions. OnTrackNY teams are committed to offering a space just for you – to be yourself and show up as you are.

Your OnTrackNY team will partner with you to determine which of our services best align with your preferences and goals. Through our work together, we hope you feel empowered to pursue what’s most important to you, whether that’s making sense of these experiences, re-engaging with hobbies, or pursuing your goals for school, work, and relationships.

At first, I needed structure so they offered me a peer support group and a brain gym to help my memory skills. And they connected me with someone who is also trans in the mental health community. They've been able to keep changing what they're doing for me as my needs have changed, like help with college applications and tips for studying.

How should I get help?

Getting support for changes in thinking and perceptions as early as possible plays an important role in recovery. Like other health challenges, the sooner you receive support for these experiences, the sooner you’ll be able to focus on pursuing your goals for school, work, and relationships.

Furthermore, the combination of early support and our services, also known as Coordinated Specialty Care (CSC), has shown to improve overall quality of life, including increasing social engagement and improving enrollment in school or employment.
