Who we serve

OnTrackNY is an early intervention program that offers Coordinated Specialty Care (CSC) services to teens and young adults across New York state experiencing non-affective psychosis. To be eligible to enroll in our program, individuals must meet the following criteria:

  • Are between the ages of 16-30 years old
  • Are experiencing symptoms of non-affective psychosis (e.g., hallucinations, delusions, unusual thoughts, etc.)*
  • Have experienced psychosis symptoms for less than 2 years*

*Exceptions: OnTrackNY Southern Tier in Binghamton, NY &  OnTrackNY CNY in Syracuse, NY may accept individuals experiencing symptoms of affective psychosis and those who have experienced symptoms for less than 5 years.
 

How to make a referral

To make a referral to OnTrackNY, contact your nearest team. If available, the following information can be helpful for making a referral:

  • Any information relevant to eligibility
  • Past treatment records

OnTrackNY teams are available to help you assess whether someone may be a fit for the program and can help navigate any potential barriers to making a referral. If they are not eligible, the team will provide a referral to appropriate care.

  2. Show compassion, listen deeply and inquire curiously.
  3. Convey warmth and hope for recovery.
  4. Challenge stigma, avoid emphasis on labels or diagnoses.
  5. Problem-solve potential barriers, such as concerns about cost and insurance coverage or involving parents, to connecting with care. 
  6. Align with the individual’s cultural beliefs and norms.
  7. Connect with families or allies, when possible, to grow their support network.
